I, the subject of personal data, give my consent in accordance with the Federal law dated 27.07.2006 No 152-FZ "On personal data" free, in my will and in my interest to the LLC "NT-Finance", located at 129128, Moscow, Severyaninsky Proyezd, ground 2, office 10, to processing with the following conditions:
1. This Consent is given to the personal data processing, both without and with the use of automated means.
2. The Consent is given to the following personal data processing:
1) Personal data that are not special or biometric: name, surname, patronymic; date of birth; place of birth; citizenship; details of the identity document; TIN; social security number (SNILS); address; contact phone numbers; e-mail addresses; place of work and office position; bank details; electronic wallets details.
2) Biometric personal data: copy of an identity document photo; photo.
3. Personal data is not to be considered as publicly available.
4. Purpose of personal data processing: Compliance with the laws and other regulations, performance of contractual obligations to the counterparty and other personal data subjects.
5. The reason for personal data processing is: Art. 24 of the Constitution of the Russian Federation; article 6 of the Federal Law №152-FZ "On personal data"; bylaws of the LLC "NT-Finance".
6. During the personal data processing the following actions will be performed: collection; recording; systematization; accumulation; storage; clarification (updating, modification); extraction; use; transfer (distribution, provision, access); depersonalization; blocking; deletion; destruction.
7. Personal data are processed up to five years after execution of contractual obligations. Upon expiry of this period, the consent shall be deemed extended for every subsequent five years in the absence of information on its revocation. Also, the personal data processing can be stopped at the request of the personal data subject. Storage of personal data recorded on paper carriers is carried out in accordance with the Federal Law № 125-FZ "On Archival Affairs in the Russian Federation" and other legal acts on archival affairs and archival storage.
8. The Consent may be withdrawn by the subject of personal data or its representative by sending a written application of NT-Finance LLC or its representative to the address specified in the beginning of this Consent.
9. If the subject of personal data or its representative withdraws the consent to personal data processing, the LLC "NT-Finance" is entitled to continue personal data processing without the consent of the subject of personal data in the presence of the grounds specified in paragraphs 2 - 11 of part 1 Article 6, part 2 of Article 10 and part 2 of Article 11 of the Federal Law No152-FZ "On Personal Data" dated 27.07.2006.
10. This Consent is valid all the time until the termination of personal data processing specified in p.8 and p.9 of this Consent.
